  1. Must be 18 years of age or older.
  2. Must benefit from the services offered, based upon Individual Program Plan objectives and long term goals.
  3. Must possess physical tolerance to sustain a six-hour, five days per week work program.
  4. Must meet eligibility requirements set by the Regional Centers in California.
  5. Individuals and their parents or significant others, as well as the Regional Center, agree to participate in the program.
  6. Must be able to administer own medication during work hours, if necessary.
  7. PPTC is not licensed to work with individuals with restricted health conditions.
  8. All individuals must be ambulatory.
  9. Individuals, in receiving our services, must be able to function within the health and safety requirements of Title 22.
  10. With the ultimate intention, which is to assist the individual in obtaining paid employment, the duration (contract period) of the Transition to Work Program will be based upon the individual’s abilities, progress, and potential employment opportunities. The expected program outcomes can be determined during the initial intake.

Upon Enrollment

  1. When an individual has been accepted into our program, the Outreach coordinator will work in conjunction with the Regional Center to coordinate placement services, areas of support, authorization of services, and transportation arrangements.
  2. All clients will be part of an initial interview via home visits or training site, that will consist of: reviewing packet information, determining functional capabilities, and collaborating with the Interdisciplinary team.
  3. A medical assessment must be submitted prior to admission.
  4. A TB test must be obtained as part of the medical assessment.
  5. PPTC implements a 30 day observation period for new program participants in order to determine if our services support their needs.
